help in posting orchids on "All Things Plants" database

The orchid moderator on "All Things Plants" has asked me to begin posting my orchids into their database, but I am a bit confused on how to fill in their specific boxes. The boxes are "Plant's Primary Common Name", "Genus", "Species", "Cultivar", and "Trade Name" (not the same as cultivar).

Use the following three orchids as examples and tell me how these five different boxes would be filled in for each of these orchid plants:
# 1 - Lc. Moochie (Bonanza x Esbetts)
# 2 - Blc. Mahina Yahiro x C. Ruth Gee 'Diamond Jubilee' FCC/AOS
# 3 - Lc. Drumbeat 'Heritage' x Sl. Orpettii 'Eastwind'
